Due to the structure of the data Ad-hoc query is not a very good tool for displaying this type of data. It is a relatively easy ABAP report to develop though. If you are running any benefits interfaces, 90% of the code already exists. Unless something has changed since I last worked on a 4.5 system (it has been almost 4 years so I am not sure) the full power of ALV reporting does not exist so the best way to write the report is the old-fashioned layout style. I know of no standard report that will give you this data.
