How do I integrate several R/3, BW and APO landscapes into Solution Manager 4.0?

I have several R/3, BW, and APO landscapes. Each landscape has its own '/usr/sap/trans', '/usr/sap/transR3', and '/usr/sap/transBW' directories.

How can I integrate these landscapes into Solution Manager 4.0? Also, do I have to create a client in another SAP system before starting any of this?
