High-risk transport best practices

What is best practice for how often transports should be applied to production for high risk, high impact changes? Should they be applied with no users on the system?
Changes should be thoroughly tested and verified before moving to production. When applying to your production region you may want to back up the production environment and then move the changes. This may be easier to facilitate when the system is quiet, without any users.
